Requirements

1. Duly Completed application form
2. Proof of Nationality – Copy of Passport bio-data page, Birth Certificate, Baptismal or any Ghanaian Identity card.
3. Two (2) Passport-size photographs, taken not more than three (3) months from application date.
4. A valid travel ticket
5. A Police Report indicating loss of passport
6. Applicant to be personally present
7. A processing fee of 21,000kz is charged.
